NOMINATE A BURLESON YOUTH TO
SERVE ON THE MAYOR'S YOUTH COUNCIL

Youth can be nominated by teachers, parents, principals, pastors, and neighbors from the community. A volunteer committee will review the nominations and choose the youth to serve as council members. The city council officially appointed the council in January.

These students are charged with establishing the by-laws, formalizing the youth council’s structure, identifying the key issues facing Burleson teens, and advising the city council on teen-related matters and other activities and projects as decided by the city council and the youth council.

The youth receving a nomination must submit the youth application form in order to be eligible for selection to the council. The deadline for all nomination and application forms has been extended to May 18, 2009.

Click on the links above for the nomination form and/or the application form. Read thoroughly and follow all instructions below before accessing online forms on this web site. You may wish to print these instructions before you complete a form. Questions about Formatta Filler are addressed via this external "Data Sheet" link. (Adobe pdf)

(for PC users)

Click on the Formatta icon above to download the file from the Formatta web site. Select "RUN" when prompted to download the "fillersetup.exe" and follow the on screen installation instructions. If using a business/corporate computer, you MUST have access/rights to install software for Filler to function. If you do not have the right to install software on your PC, please "Save" (to a familiar location on the PC) instead of "Run" and contact your IT support to install this software. After installation, Formatta Filler will launch to the Formatta registration screen. Registration is not required but will provide additional support from the software provider. You may simply choose to close the window at this point. Now you are ready to open either the Mayor's Youth Council Nomination form or the Youth Application form above..

(for Mac users)

Click on the Formatta icon above to download the file. When promoted, save to the application folder on your computer. Now you are ready to open either the Mayor's Youth Council Nomination form or the Youth Application form above. Choose Formatta Filler.

Once you have filled out the form, choose Save As to save to your computer. You may then open your email program and send this form as an attachment.
