2009 Citizen Evaluation of the
Burleson Police Department

Dear Burleson Resident:

We are compiling information about the Burleson Police Department’s level of service, and we would appreciate your cooperation in filling out the questionnaire below.  We are always seeking to improve the quality of our service to our community.  Your answers to our questions will be valuable in helping us assess the quality of our service and to plan for the future.

Please take a few minutes to provide us with the information requested on the survey form.  Rate the specific services, as you perceive them.  If you were fortunate enough not to have required the services, please rate them as you believe the majority of the community perceives them.  The last entry is included for your convenience so you may rate any other service not listed.  Any additional information and comments you might provide will be appreciated. Your personal information is optional but please provide your street address so the survey will accurately represent City of Burleson residents.

This survey can be completed in two ways. It can be filled out and submitted on-line or printed off, completed and returned either by mail, to any Burleson Police Officer or employee, or dropped off at any City Facility. The deadline for submitting the survey is November 23, 2009.

(for PC users) Click on the Formatta icon above to download the file from the Formatta web site. Select "RUN" when prompted to download the "fillersetup.exe" and follow the on screen installation instructions.  If using a business/corporate computer, you MUST have access/rights to install software for Filler to function.  If you do not have the right to install software on your PC, please "Save" (to a familiar location on the PC) instead of "Run" and contact your IT support to install this software.   After installation, Formatta Filler will launch to the Formatta registration screen.  Registration is not required. Close the window and return to the City of Burleson website to open the form above.

(for Mac users) Click on the Formatta icon above to download the file. When promoted, save to the application folder on your computer. Go back to this page to open the online form You will be asked how you want to open the form. Choose Formatta Filler. Once you have filled out the form, choose Save As to save to your computer. You may then open your email program and send this form as an attachment to pibsen@burlesontx.com.
