Please fill out one of the forms below if your request is not of an emergency nature. Please do not use these forms to report the following:

• Police and Fire emergencies - report by telephone to 911.
• Missing or damaged regulatory signs (stop signs, yield signs, etc.), water main breaks and sewer stoppages or overflows - report to 817-426-9830, during regular business hours and to 817-426-9903 after regular business hours.
• Street light problems in areas that are served by Oncor - call 888-313-4747 or submit an online request at www.oncorgroup.com/community/streetlights/default.aspx.

In order to utilize the service request forms you must install Formatta Filler software on your PC. Follow the instructions below. You may wish to print these instructions before you complete a form. Questions about Formatta Filler are addressed via this external "Data Sheet" link. (Adobe pdf)

Click the Formatta Filler link above to download the application. Select "RUN" when prompted to download the "fillersetup.exe" and follow the on screen installation instructions.  If using a business/corporate PC, you MUST have access/rights to install software for Filler to function.  If you do not have the right to install software on your PC, please "Save" (to a familiar location on the PC) instead of "Run" and contact your IT support to install this software.   After installation, Formatta Filler will launch to the Formatta homepage and prompt you for registration of the free product.  Registration is not required but will provide additional support from the software provider.
