The City of Burleson is now able to provide citizens the ability to securely and safely submit personal data via the Internet through this web site. In order to utilize the City of Burleson web forms you must install Formatta Filler software on your PC or Mac.  Formatta Filler is much like Adobe® Reader.  Formatta Filler version 7 is designed for Microsoft Windows (and Mac OS X v10.3) and Internet Explorer 6.x is recommended for it to properly function. It uses high level encryption and security technologies preventing anyone except the submitting person and the recipient to view/open the form. 

Should you choose not to submit forms online, we will continue to provide forms in Adobe pdf format. (see the Forms page) These forms can be printed and then faxed, mailed, or submitted in person. Please keep in mind the Formatta Filler forms can also be printed if you choose not to submit them online.

Read thoroughly and follow all instructions before accessing online forms on this web site.  You may wish to print these instructions before you complete a form.

(for PC users) Click on the Formatta icon above to download the file from the Formatta web site. Select "RUN" when prompted to download the "fillersetup.exe" and follow the on screen installation instructions.  If using a business/corporate computer, you MUST have access/rights to install software for Filler to function.  If you do not have the right to install software on your PC, please "Save" (to a familiar location on the PC) instead of "Run" and contact your IT support to install this software.   After installation, Formatta Filler will launch to the Formatta registration screen.  Registration is not required but will provide additional support from the software provider. You may simply choose to close the window at this point. Now you are ready to open one of the online forms below.

(for Mac users) Click on the Formatta icon above to download the file. When promoted, save to the application folder on your computer. Go back to this page to open one of the online forms below. You will be asked how you want to open the form. Choose Formatta Filler. Once you have filled out the form, choose Save As to save to your computer. You may then open your email program and send this form as an attachment.
